Connections Defined

The Connections Program was created in response to low-income and health care needs of rural communities in Kentucky. The Kentucky United Methodist Home Community Services received a grant from the Office of Rural Health Policy to improve the health of rural area residents in Kentucky, thus Connections was born! Currently, Connections serves two counties; Madison and Anderson. Connections provides a link between needy families and health care resources. Program Coordinators offer in-home Tobacco and Diabetes-Education, Nutritional Counseling, and Mental Health Counseling to individual families. Coordinators also work with community agencies and schools to provide Smoking Cessation, Nutritional Workshops, Healthy-Image groups and Arthritis Pain Management Support groups. Connections is a unique program, based on a holistic approach to health and wellness.
